Portland District Health is reaching into the community with a variety of programs being offered to people where they live, work and play.

A range of allied health and community health services are being delivered by PDH both within and outside of the hospital precinct.

PDH Director of Primary and Community Health, Fiona Heenan, said the community-based services were being presented as part of strong community partnerships.

PDH believes in flexible delivery of services and is keen to move into the community whenever possible, Ms Heenan said.

We're taking services to where people are. This is more convenient and gives people an opportunity to access services in their preferred environment, she said.

It helps PDH to ensure our services are being well utilised by local people.

Some of the community-based programs include occupational therapy, rehabilitation with an exercise physiologist and dietitian, speech therapy, support for Home and Community Care staff and diabetes education.

PDH delivers programs in conjunctions with organisations including Dhauwurd-Wurrung Elderly and Community Health Services Inc (DWECH), Windamara Aboriginal Corporation, Glenelg Shire Council, YMCA, Portland Bay School, Kyeema, and Heywood Rural Health.

PDH is always looking at ways to better connect with the community and provide services where they are convenient for everyone,” Ms Heenan said.

Ms Heenan said services were confidential and presented in environments safe for staff and clients.

Some services are also available in homes.

PDH is keen to hear from local groups, organisations or individuals who are interested in receiving existing services or would like new services to be offered.

We have developed strong partnerships with many local organisations and will continue to look for opportunities to provide more services to support people in the community, Ms Heenan said.

PDH and Active Health Portland have a broad range of allied and community health services available include physiotherapy, counselling, diabetes education, dietitians, exercise physiology, occupational therapy, social workers and district nursing among others.
