South West Healthcare is committed to protecting your privacy, while providing you with the highest level of service. Personal information is required to ensure that each health professional involved in your care has all the necessary information to plan your treatment.
As well as personal details such as name, address and contact details, we record information relating to diagnosis, treatment and outcomes of treatment for all patients or service clients, and for billing purposes if applicable, including private health insurance, Workcover, TAC etc.

Information is updated each time you attend for health care, to ensure accuracy and completeness. It is customary for clinicians to involve your next-of-kin and other family members in your care plan, unless you request otherwise.

If you receive care from another health service, they may request copies of relevant health information to be forwarded to them to assist in your treatment. We only release health information that is relevant to your current condition and necessary for your continuing care.

South West Healthcare protects your privacy by ensuring personal information is secured through strict policies regarding access, handling, storage and disposal. All staff are bound by a strict code of conduct in maintaining confidentiality of your personal information.
