The District Nursing Service is provided by qualified Registered Nurses. The service is designed to meet the diverse health needs of the community.
This service provides health care and support to clients and their families in the comfort and security of their own home. The District Nurse has a great deal of experience and knowledge and works in partnership with your local GP and other health providers. This includes:
- Case Managers
- Continence Nurse
- Physiotherapy
- Occupational Therapist
- Dietician
- Podiatrist
- Home Help
- Meals on Wheels
What does the District Nurse provide?
- Wound Management
- Injections
- Observations Monitoring
- Blood Collecting
- Clinical Assessment
- Bowel and Bladder management including stomal, continence and catheter care
- Health education for clients and families
- Supportive Health Care
- Palliative care in the home
- Care planned with yourself and your doctor
District Nursing Service Hours
District Nursing Services are available from Monday to Friday from 7.30am-3.30pm There is provision of a weekend and after hours service to clients if required.
Eligibility
- The District Nursing Service is available to frail older people and people with disabilities who are experiencing difficulties:
- Pensioner
- Department of Veteran Affairs or Gold Card Holder
- Post Acute Care
- Palliative Care
Accessing the District Nursing Service
Clients can be referred to the District Nursing Service from hospital, community support services, carers, GP’s or self– referral. All clients have the right to refuse a service and/or appeal a service provision decision. This will not effect the clients’ access to the services in the future.
