What are Community Packaged Care Programs?

The packaged care programs available from Moyne Health Services are as follows:

Community Aged Care Packages (CACP’s)

This package is for frail older people, aged 70 years and over or 50 years and over for people of Aboriginal and Torres Strait Islander descent, living in the community, who require provision of services because of their complex needs.

These people would otherwise be assessed for at least low level residential care.

Moyne Health Services is funded to provide 20 CACPS packages.

Extended Aged Care at Home Packages (EACH)

This package is for frail older people, aged 70 years and over or 50 years and over for people of Aboriginal and Torres Strait Islander descent, with complex care needs who wish to remain living in their own home.

These people would otherwise be assessed as eligible for high level residential care.

Moyne Health Services has funding for 5 EACH packages.

Eligibility

To be eligible for either a CACP’s or EACH package you must be assessed by an Aged Care Assessment Team (ACAT).

ACAT Assessments are free of charge. Information on ACAT is available from doctors, hospitals, community service providers or the Aged and Community Care Information Line on 1800 500 853

Fees

You can be asked to pay a fee for any of our community packages. The amount charged forms part of an agreement between yourself and Moyne Health Services.

Accessing Community Care Services

Community Care Services aim to be available to everyone who is entitled to them. Referral to Community Care Services may be from the hospital, community support services, carers, GPs or self– referral.

All clients have the right to refuse a service and/or appeal a service provision decision. This will not affect the clients access to the services in the future.
