Home Nursing

The Lorne District Nursing service provides nursing care to the frail, aged and people with disabilities in their own home. The district nurse will also assist to coordinate services and supports to ensure remaining in your own home is as enjoyable and safe as possible.

Referrals

It is not necessary to have a referral to see the District Nurse. A client, carer or other family member can request a visit from the district nurse.

You may also receive a visit from the district nurse as part of a coordinated discharge plan after a stay in a public hospital.
